Consultation services provided by a Qualified Professional (QP) for Community First Services and Supports (CFSS) in Minnesota involve offering guidance, oversight, and support to individuals receiving CFSS. The QP helps participants understand their service options, develop a person-centered plan, and manage their services.
Essential Job Functions:
• Adhere to all Arc Northland Policies and Procedures as outlined in Arc Northland’s Employee Handbook.
• Must pass Department of Human Services Background Study
• Assist Consultation Services participant or their participant representative to create their service plan based on the needs of the participants service agreement.
-Assessment and Planning: Assisting participants in identifying their needs and developing a personalized care plan.
-Guidance: Providing expert advice on selecting and managing direct support workers and other service-related decisions.
-Monitoring Services: Ensuring that the participant’s services are delivered in accordance with their plan and in compliance with CFSS guidelines.
-Support and Training: Offering training and resources to participants on how to manage their services effectively and use CFSS to meet their goals.
• Document communication with participants and/or participant representatives
• Document time recording with participants and/or participant representatives
Preferred Skills:
• Knowledge of person-centered supports and services
• Knowledge of personal care services and health care
• Knowledge of physical mental and cognitive disabilities and their characteristics
• Knowledge of computer applications and computer programs
• Ability to communicate effectively both orally and in writing
• Ability to establish and maintain effective working relationships
Required Qualifications:
Have at least one year of experience working directly with people who have an intellectual disability or other developmental disability and have an associate’s degree or higher in a discipline associated with at least one of the following fields of study:
-Human behavior
-Human skill development
-Humans and their cultural behavior
-Human condition
-Any other study of services related to basic human care needs
Education substitution. Staff working directly with people substitute one of the following for a bachelor’s degree:
• One year of full-time experience providing direct services to people with disabilities or people older than age 65.
• Experience coordinating or directing services for people with disabilities or people older than age 65, including self-directed services.
• Experience coordinating their own services.
Must be proficient with technology.
Must have access to reliable transportation.
